Class List Information Form

Each year, we gather contact information for every family.  This help us build the main PTO mailing list as well as class contact lists.

Please fill out the following information so that we can keep you informed with Class Party info, PTO events and opportunities to get involved. This information will be shared with the other parents in your Child’s classroom so they can reach out for possible play dates, the Homeroom Parents for class party planning and other communications. Our teachers aren't able to share this information with us so this will be the only way Homeroom Parents can communicate. Thank you!
